Vision for Life Plan

The Lifetime Vision Plan offers eligible LASIK patients additional peace of mind after vision correction surgery. If your vision changes over time, the plan may cover a future laser enhancement at no additional procedure cost, provided you meet the medical and program requirements.

What Is The Vision for Life Plan

The Vision for Life Plan is an optional enhancement program available to eligible LASIK/PRK patients before their original procedure.

LASIK and PRK are laser vision correction procedures that reshape the cornea to reduce dependence on glasses or contact lenses. LASIK creates a thin flap in the cornea, while PRK reshapes the cornea without creating a flap. This makes PRK a common option for firefighters, military personnel, and others whose work or lifestyle may involve physical contact or an increased risk of eye injury.

For many patients, vision correction results remain stable for years. However, natural changes in the eyes can lead to prescription changes over time, including nearsightedness, farsightedness, or astigmatism. In some cases, an additional laser procedure, called an enhancement, may help address these changes.

The Vision for Life Plan helps cover the surgical cost of an eligible enhancement when all program requirements are met. While the plan does not guarantee that your vision will never change or that you will never need glasses again, it provides qualified patients with a pathway to be re-evaluated and, when appropriate, treated by an American Vision Partners surgeon.

How the Vision for Life Plan Works

1

Enroll Before Your LASIK/PRK Procedure

The Vision for Life Plan must be purchased before your original LASIK/PRK procedure. During your consultation and pre-operative planning, your care team will explain the program, review the eligibility requirements, and answer your questions.

The one-time enrollment fee is $275 per eye, or $550 total for both eyes.

2

Complete Your Recommended Post-Operative Care

After LASIK/PRK, it is important to attend your scheduled follow-up appointments, use prescribed medications as directed, and follow your surgeon’s recovery guidance.

These visits allow your surgeon to monitor healing, evaluate your visual outcome, and make sure your eyes are recovering as expected. To remain eligible for the Vision for Life Plan, patients must complete all required post-operative care during the first 12 months after surgery.

3

Stay Connected Through Annual Eye Exams

After the first year, plan members must have an eye exam every year to keep their eligibility active.

Annual exams may be completed with an American Vision Partners provider or another qualified eye care provider. When exams are completed outside the American Vision Partners network, patients should keep their records and provide them if requested.

Routine eye exams are not included in the Vision for Life Plan and are billed separately through applicable vision insurance, medical insurance, or self-pay arrangements

4

Schedule an Evaluation If Your Vision Changes

A change in vision does not automatically mean you need an enhancement. It may be caused by a prescription shift, dry eye, cataracts, age-related reading changes, an eye condition, or another factor that requires a different type of care.

When you notice a change, schedule an eye exam with an American Vision Partners provider. Your eye doctor or refractive surgeon will evaluate your eyes, determine what may be affecting your vision, and discuss your options.

5

Receive an Eligible Enhancement When Appropriate

When a surgeon determines that a laser enhancement is medically appropriate, you remain a candidate for treatment, and all program requirements have been met, the enhancement procedure may be performed by an eligible American Vision Partners surgeon without an additional surgical charge.

Who May Be Eligible for Vision for Life?

Eligibility begins before your original LASIK/PRK procedure. To enroll and remain eligible, patients must generally:

  • Purchase the Vision for Life Plan before their LASIK/PRK procedure.
  • Have their original LASIK/PRK procedure performed by an eligible American Vision Partners surgeon.
  • Meet the surgeon’s vision, prescription, and eye health requirements before surgery.
  • Be correctable to 20/30 vision or better before LASIK/PRK.
  • Complete all required follow-up care during the first 12 months after surgery.
  • Remain current on financial obligations related to the original LASIK/PRK procedure.
  • Complete an annual eye exam every year after the first year of post-operative care.
  • Continue to be a candidate for laser vision correction if an enhancement is considered.
  • Be evaluated by an American Vision Partners provider before any enhancement is performed.

A surgeon’s recommendation is always based on the individual patient. Even when a prescription change has occurred, an enhancement may not be appropriate for every eye.

What Does The Vision For Life Plan Cover

The Vision for Life Plan is designed to cover eligible laser enhancements related to the original refractive error treated with LASIK/PRK.

This may include a future change related to:

  • Nearsightedness
  • Farsightedness
  • Astigmatism

For an enhancement to be covered, the change must be evaluated by an American Vision Partners provider, the patient must meet all program requirements, and an eligible surgeon must determine that another laser procedure is medically appropriate.

What Is Not Covered?

The Vision for Life Plan is not a replacement for routine eye care, medical treatment, health insurance, or vision insurance. It is designed specifically for qualifying laser enhancements related to the original LASIK/PRK procedure.

The plan does not cover vision changes caused by:

  • Cataracts
  • Glaucoma
  • Macular degeneration
  • Diabetic eye disease
  • Retinal conditions
  • Eye injuries or trauma
  • Presbyopia, or age-related difficulty with near vision
  • Vision changes related to cataract surgery or another eye procedure
  • Other eye diseases or health conditions that may affect vision

The plan also does not cover prescription medications, glasses, contact lenses, medical devices, travel expenses, routine eye exams, or other medical costs. Enhancements performed outside the American Vision Partners network are not covered.

Vision for Life Frequently Asked Questions

No. LASIK can reduce dependence on glasses and contact lenses, but it does not stop natural aging or future changes in your eyes. The Vision for Life Plan does not guarantee perfect vision for life. It provides coverage for a qualifying laser enhancement when one is medically appropriate.

No. Enrollment must happen before your original LASIK/PRK procedure.

No. Your surgeon must first determine what is causing the change in vision. Some changes may be related to dry eye, cataracts, presbyopia, or another eye condition rather than a correctable refractive change. Your eye health and corneal measurements must also support another laser procedure.

Another qualified American Vision Partners surgeon can evaluate your eyes and determine whether you remain eligible for an enhancement.

There is no fixed deadline for an eligible enhancement. Benefits remain available as long as you continue to meet the program requirements, including annual eye exams, and an American Vision Partners surgeon determines that an enhancement is medically appropriate.

Keep Your Vision Care Connected

Your eyesight can change at different points in life. The Vision for Life Plan gives eligible LASIK/PRK patients a way to stay connected to experienced eye care providers, monitor their long-term vision, and access a future laser enhancement when it is appropriate for their eyes. Schedule a LASIK/PRK consultation to learn whether you may be a candidate for LASIK/PRK and to discuss whether the Vision for Life Plan is right for you.
